What your pipeline actually did
Built from two instruments read side by side: your call recordings and your CRM.
Neither is the pipeline on its own. Recordings know what was said; the CRM knows
what it became; the gaps between them are where deals die quietly.

What this caught this period
A deal everyone believed had gone quiet was scored as dead in the weekly review. The recording layer showed the prospect had personally booked a follow-up call twelve days earlier. No system held the date; the deal was alive the whole time. The call went ahead as booked.
The CRM showed a second deal at Proposal Stage. The recordings and the owner's own account showed it had already been lost weeks earlier. The pipeline forecast was carrying a dead deal as live.
Two CRM threads turned out to be the same person under two different names. One half was being aged toward write-off while the other half sat at Proposal Stage.
Commercial terms stated on two different sales calls, eight days apart, did not match: the same guarantee was described one way to one prospect and a materially cheaper way to another. Flagged before either signed.
The next-step board
An agreed next step that passes with no recorded call is the earliest
observable sign of a deal dying. It is a question to ask, never a verdict: the meeting
may have moved to email or gone unrecorded.

Not dead on this evidence: may be moving over email, or may be quietly parked. The point is nobody can currently tell.
